Rehoboth Elderly Care and Companionship LLC is authorized by the New York State Department of Health’s Office of Health Insurance Programs, Division of Long-Term Care, to provide services under the Nursing Home Transition and Diversion (NHTD) Medicaid Waiver.
NHTD Waiver Program
The NHTD waiver program is a home and community-based initiative designed to assist New York’s Medicaid-eligible seniors and individuals with physical disabilities. It enables participants to receive comprehensive services while residing in community-based settings, rather than in nursing homes, congregate care facilities, or other institutions. Participants are empowered to make decisions about the services they receive, select their service providers, and set their personal goals.
Home and Community Support Services (HCSS)
Home and Community Support Services (HCSS) are the combination of personal care services (Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)) with oversight and/or supervision as a discrete service. Services are provided primarily in a participant’s home. HCSS is provided to a waiver participant who requires assistance with personal care services tasks and whose health and welfare in the community is at risk because oversight and/or supervision of the participant is required even when no personal care task is being performed. Services may compliment, but not duplicate, other services.
